This section is to request advertising. You are required to give all pertinent information regarding your event in order for us to advertise. Please note that any ads or Artwork will be subject to the approval of the church staff.

Child Care
This section is to be filled out if you will need child care for your event. The church will only provide child care during regularly scheduled service times. We only provide child care for children 4 year old and younger.

Financial Responsibility
This section is to plan the financial responsibilties of the event. Approval for all funds spent from the churches budget must be approved first (by filling out a Purchase Order request) even if you have an approved budget amount for your event.






Please not that a Ministry Event Request Form must be filled out for each fundraiser.
Audio Visual Needs
This section needs to be filled out for any equipment you may need for your event as well as someone to run the equipment. Although you make a request for the equipment, please know that it is on a first come first serve basis and the equipment may be in use by another ministry.

Set Up and Clean Up
This section pertains to set up, room requested and opening and closing the building.
